The Adult Family Care Program provides foster care for any adult in Massachusetts who is on standard MassHealth and who has a medical or psychosocial need requiring assistance with their healthcare management and a few basic activities of daily living in order to remain in the community. This program provides an option to living alone and serves adults such as: elders; individuals who are developmentally disabled; those who are living with HIV/AIDS, Hepatitis C, or substance abuse issues; and those with mental health challenges.

We have families and individuals in the Plymouth are who are interested in welcoming eligible participants into their homes.
Participants who qualify for our program may be able to find their own caregiver (provided that they are not related and are sharing living quarters). Those who qualify may be eligible to receive a $20 per-day, tax-free stipend for assisting their house mate.

The Greater Plymouth AIDS Consortium (GPAC) Contact Information: (508) 747-2211
The Greater Plymouth AIDS Consortium (GPAC) is an HIV services planning body, comprised of local consumers and providers, who provide guidance to the Massachusetts Department of Public Health in the administration of local HIV/AIDS services. GPAC may conduct local needs assessments, identify service barriers and gaps, sponsor educational programs or work on other relevant projects. Funded by the Massachusetts Department of Public Health, the Consortium meets every two months and is open to service providers and all people who are effected by or infected with HIV/AIDS.

The Consumer Advisory Board (CAB) of the consortium is a group of local consumers (persons living with HIV or the parent of an HIV-positive child under 18), who advise the Consortium about consumer needs in the region. Also funded by the Massachusetts Department of Public Health, the Greater Plymouth AIDS Consortium CAB meets monthly.
